Kelly Preston’s death certificate reveals she died at her family’s Florida home following a battle with breast cancer, according to US Weekly.
John Travolta’s wife passed away on July 12 at the age of 57 after a two-year secret battle.
According to the certificate obtained by the publication on Tuesday, August 18, her body was transferred from the Clearwater, Florida home she shares with Travolta to a funeral home.
She was cremated at the Brooksville Crematory in Brooksville, Florida, the report notes.
The “Medium” actress is survived by her husband of nearly three decades and their children, 20-year-old Ella and 9-year-old Benjamin.
The couple also shared a son, Jett, who died in 2009 at the age of 16 following a seizure.
Ella remembered her mother in a heartfelt tribute on Instagram after her death.
“I have never met anyone as courageous, strong, beautiful and loving as you,” she wrote.
“Anyone who is lucky enough to have known you or to have ever been in your presence will agree that you have a glow and a light that never ceases to shine and that makes anyone around you feel instantly happy. Thank you for being there for me no matter what,” she added.
The “Grease” actor also remembered his wife on Instagram writing, “She fought a courageous fight with the love and support of so many. Forgive me in advance if you don’t hear from us for a while. But please know that I will feel your outpouring of love in the weeks and months ahead as we heal.”
